Chart Reviewer
PRINCIPAL DUTIES AND RESPONSIBILITIES
• Print daily physician /charting report
• Attend monthly meetings with the Revenue Cycle Team as requested.
• Review and analyze medical documentation within provider charts for processing.
• Meet daily chart review production requirements.
• Serves as resource regarding insurance resolutions and charting questions
• Assigns charges to diagnoses and procedures as stated by the physician are valid and complete.
• Evaluating clinical documentation to ensure quality, completeness and consistency as it relates to code selection and reimbursement.
• Ensuring documentation compliance with all insurance mandates, quality and reporting requirements.
• Follow all regulations and guidelines set by Medicare, state programs, and HMO/PPO.
• Perform chart reviews consistently and within the time periods(s) established by management
• Assist in completion and updating of quality measures within insurance portals.
• Assist in updating patient records for preventative measures, answering phones, scheduling appointments, and other medical office related tasks.
• Commits to maintaining confidentiality of patient charts, medical information and physician information including profiles and fees. (HIPPA Compliance)
• Identify problem accounts and brings them to the attention of appropriate personnel who can fix the problem.
• Demonstrates excellent customer services skills and team work in order to work with staff in a tactful, courteous, and professional decorum.
• Enroll in continuing education courses to stay current in medical field.
• Protects patient’s rights by maintaining confidentiality of personal and financial information.
• Receive and convey messages in writing, electronically and verbally to the appropriate person.
• Keeps the practice administrator informed of any customer complaints, system problems, or any situations that affect the day-to-day operation of the clinic in a timely manner.
• Respond to emails that arrive using our general office email address.
• Uphold the positive reputation of our medical office by always treating visitors in a friendly, welcoming, and compassionate manner.
• Contributes to team effort by accomplishing related results as needed.
• Performs additional job duties as assigned.
• Attends monthly staff meetings.
• Is punctual and prompt for work.
• Follows the policies and procedures for sick and unscheduled absences.
• Adheres to the policies and procedures for Dress Code.
• Completes in-services and health exams as required

JOB SPECIFICATIONS: DESCRIPTION
1. EDUCATION High School Diploma

2. LICENSURE Completion of a certificate program or commensurate experience. (medical billing, medical scribe)

3. EXPERIENCE
Two years’ previous experience as a medical chart reviewer or in a related healthcare support position
Computer skills, Microsoft Office Suite (Word, Power Point, Outlook, and Excel); working knowledge of billing software a plus

5. INTERPERSONAL SKILLS
Excellent written and verbal communication skills. Ability to build relationships and quickly develop trust with patients and healthcare professionals.
Ability to work independently or as part of a team.
Organizing, multi-tasking, and flexible to handle fluctuating workloads and tasks.
Self-motivated and self-directed; able to work without supervision.
Strong computer skills in data entry, coding, and knowledge of Electronic Medical Record Software; Microsoft Office Suite
Able to analyze problems and strategize for better solutions
Understanding of medical terminology, anatomy and physiology
Accurate and precise attention to detail
Goal- oriented, organized team player
Knowledge of medical terminology, abbreviations, techniques and operative procedures; anatomy and physiology; major disease processes; pharmacology; and the metric system to identify specific clinical findings, to support existing diagnoses, or substantiate listing additional diagnoses in the medical record.
Requires skill in the use of a wide variety of office equipment as required.

Strong customer service skills and comfortable answering both patient and insurance company questions

6. PHYSICAL EFFORT

7. HOURS OF WORK Ability to concentrate and maintain accuracy
Frequently required to retrieve stacks of medical records loose reports weighing up to 15 pounds
Occasional bending, stooping, kneeling, crouching, reaching.

Monday-Friday with extended hours as needed.


8. HAZARDS
Exposure to chemicals, machinery, and blood borne pathogens that requires the use of personal protective equipment, i.e., masks, gowns, & gloves.



9. COMPETENCE FOR SPECIFIC PATIENT POPULATION:
Infant, Pediatric, Adolescence Adult and Geriatric

10. INFECTION CONTROL
Ability to comply with department specific controls implemented to reduce the risk of acquiring and transmitting infections within the health care settings. The controls may include, but are not limited to, immunity to certain vaccine preventable diseases, screening for tuberculosis infection or diseases, the ability to utilize indicated personal protective equipment and attendance at appropriate training.
